It means leaving home takes considerable, taxing effort or requires help or a device — not that you can never leave. Medical appointments, religious services, and occasional outings are all allowed.
That's our default. Consistency is better medicine, so we schedule for it — and when coverage is unavoidable, the covering clinician gets a full handoff first.
